As part of the grant work, the Co-Op is conducting facility outreach to their respective members and clients to recruit facilities to participate in the grant pilot. The outreach effort includes data collection through checklists to determine participant viability. The information collected may include highly sensitive details regarding a facility’s health information systems and data transfer protocols and will only be used for business specifically related to the grant effort unless given express consent by the primary designated facility representative.
